Karen and Phil's relationship reaches a crisis point when Phil cheats on Karen with the new girl at work and Karen falls pregnant with Phil's child. Karen must also deal with a problem patient whose troubled life reflects her own; meanwhile Phil is caught in a professional vacuum, continually being passed up for promotion by his manager. Karen's doubts about Phil's commitment to her and their future family force her to take drastic steps.
